Benton County has two health unit locations that can issue certified birth certificates. Both are part of the Arkansas Department of Health system. Since 2018, both locations can issue certificates for births that happened anywhere in Arkansas, not just Benton County births. So if you were born somewhere else in the state, you can still use the Bentonville or Siloam Springs office to get your certificate.

The Benton County Health Unit in Bentonville is at 2203 S.W. I Street, Bentonville, AR 72712. Call (479) 986-1300 to reach this location. It serves the eastern part of Benton County, including Rogers and Lowell. Hours are Monday through Friday, 8:00 AM to 4:30 PM.

The Benton County Health Unit in Siloam Springs is at 1010 W. Pleasure Avenue, Siloam Springs, AR 72761. Phone is (479) 373-7000. This location serves the western part of the county including Bella Vista, Gravette, Decatur, Gentry, and surrounding areas. Same hours apply.

Same-day service is available at both locations when you arrive before 3:00 PM with complete documents. How to Request a Benton County Birth Certificate

Getting a certified Benton County birth certificate is straightforward once you know which method fits your situation. In-person visits at either health unit are fastest. Mail and online requests work when you cannot visit during business hours.

In person: Visit either the Bentonville or Siloam Springs health unit. Bring your government-issued photo ID, any documents that show your relationship to the person named on the certificate, and your payment. For requests with complete information submitted before 3:00 PM, staff can generally provide the certificate the same day.

Online: Use the VitalChek Arkansas vital records portal, the state's authorized online ordering service. You pay the standard state fee plus a VitalChek service charge. Your certificate gets mailed to your address. Allow extra time for processing and delivery.

By mail: Complete the birth certificate request form, include a copy of your photo ID, explain your relationship to the person on the record, and send a check or money order payable to Arkansas Department of Health. Mail to: Division of Vital Records, 4815 West Markham Street, Slot 44, Little Rock, AR 72205. Mail turnaround is typically two to four weeks.

Who Can Access Benton County Birth Records

Arkansas restricts access to birth records less than 100 years old under Arkansas Code 20-18-305. The rule applies everywhere in the state, including Benton County. You cannot request a copy of someone else's birth certificate unless you qualify as an authorized person.

Authorized requesters include the person named on the record (if they are 18 or older), a parent listed on the certificate, a legal guardian, an adult child of the person, a sibling, a spouse, or an attorney or agent with written authorization. Government agencies conducting official business can also access records for lawful purposes.

If you are requesting on behalf of someone else, bring written authorization from that person or legal documentation of your relationship. Without the right paperwork, the health unit staff cannot legally release the record to you, and you will still be charged the search fee if they have to conduct a search. Ask ahead of time if you are unsure what to bring.

Benton County Birth Certificate Fees

State fees apply equally at both Benton County health units and at the central state office in Little Rock. The first certified copy costs $12. Each additional copy of the same record, ordered at the same time, costs $10. If the Department of Health conducts a search and does not find a matching record, a $12 non-refundable search fee is still charged.

The non-refundable search fee is one of the most common surprises for first-time requesters. Even if you believe a record should exist, incorrect details like a wrong birth year or a name spelled differently than expected can trigger a failed search and a lost $12. Before submitting, verify the full legal name, birth date, and parents' names as carefully as you can.

Both Benton County health units accept cash, personal check, and credit or debit cards at the counter. Mail-in requests should be accompanied by a check or money order only.
