Where to Get Fort Smith Birth Records

The Sebastian County Health Unit is the primary local office for Fort Smith birth certificate requests. Walk-in service is available during regular business hours, and same-day processing is common for standard requests. The unit is part of the Arkansas Department of Health network and can access the full state vital records system.

The state's central office is the Arkansas Department of Health Division of Vital Records at 4815 West Markham Street, Slot 44, Little Rock, AR 72205, phone (501) 661-2174. It is open Monday through Friday, 8:00 AM to 4:30 PM. Arrive before 3:00 PM and you can generally walk out the same day with a certified copy.

The City of Fort Smith does not issue birth certificates. Per the city's public records page, birth and death records should be requested from the State of Arkansas Department of Health at 1-800-462-0599. The city handles FOIA requests through its own portal, but vital records are a state function. Marriage records go to the Sebastian County Clerk at 479-782-5065, and divorce records go to the Sebastian Circuit Clerk at 479-782-1046.

How to Request a Fort Smith Birth Certificate

In-person service at the Sebastian County Health Unit is the fastest option. Bring your photo ID, the completed request form, and payment. Most walk-in requests are processed the same day. If you arrive after the early afternoon cutoff, you may need to return the next business day.

Online requests use VitalChek, the state-authorized online ordering service. Fill out the order form, pay by credit card, and the certificate ships to your address. VitalChek processes orders within a few business days after identity verification is complete.

Mail orders go to the Division of Vital Records at 4815 West Markham Street, Slot 44, Little Rock, AR 72205. Send a completed application, a copy of your government-issued photo ID, a check or money order payable to the Arkansas Department of Health, and a self-addressed stamped envelope if you want the certificate returned by mail. Processing takes longer for mail orders than for walk-in or online requests.

The Arkansas Department of Health website has the forms you need and full instructions for each request method.

What You Need to Apply

Have these items ready before you go to the office or submit your request. Incomplete applications cause delays.

  • Valid government-issued photo ID
  • Completed ADH vital records request form
  • Full legal name on the record, date of birth, and city of birth
  • Your relationship to the person named on the certificate
  • Payment of $12 for the first copy ($10 for each additional same-order copy)
  • For mail: a copy of your ID and a self-addressed stamped envelope

Note: If the person on the record is not you, be ready to explain and document your qualifying relationship before staff can process the request.

Fort Smith Birth Certificate Fees

The fee is $12 for the first certified copy. Additional copies of the same record ordered at the same time cost $10 each. The $12 fee covers the search and is not refunded even if no matching record turns up. Pay by cash, check, or credit card at in-person offices. Use a check or money order for mail requests. VitalChek accepts credit cards but may charge a small service fee on top of the state's base rate.

If you know you need multiple copies for different purposes, such as one for a passport application and one for a legal matter, order them all together. That way you pay $10 each for the extras instead of $12 per order on separate requests.

Who Can Get Fort Smith Birth Records

Arkansas Code 20-18-305 sets the rules for who can receive a certified copy of a birth record that is less than 100 years old. Qualifying persons include the subject of the record (if 18 or older), a parent or legal guardian, a spouse, an adult child, or an attorney acting for a qualifying person. Government agencies with a lawful purpose are also authorized.

Records that are 100 years old or older are open to the public. No qualifying relationship is needed for those. Fort Smith's birth records go back to 1881, so a large portion of the early collection is now fully public. This is particularly useful for genealogists and family historians researching Fort Smith roots.

You must provide valid photo ID for any request. For records under 100 years, staff will also ask about your relationship to the subject.
